📋 Required to Begin Practicing & Receive Equipment
A birth certificate on file and a completed credit card authorization are both required before your player can begin practicing and receive league equipment. Please complete this form in full before equipment distribution day.
All practice and game equipment must be returned (except the mouth guard) to the Sebastian Sharks Youth Football and Cheerleading Association. Failure to return equipment with result in a $450 charge!!
We use Zeffy, a fundraising and payment platform built specifically for nonprofits.
One of the reasons we chose Zeffy is that they do not charge processing fees to nonprofit organizations like the Sebastian Sharks Youth Football & Cheerleading Association. This allows more of your registration fees to directly support our athletes and programs. 🦈🏈📣
